- Official form of ID (State ID, Driver’s License, Passport, etc.)
- All prescription medications
- Inhaler for respiratory conditions, Epi-pens, Insulin pens, other specialized medications. Speak with staff before bringing.
- Your checkbook, credit card or debit card to pay for treatment incidentals. We require a credit card to be put on file for pharmacy copays. Additionally, if you need nicotine products or toiletries, we have them available for purchase.
- A list of names, addresses and phone numbers of those individuals or organizations you wish to have involved in your treatment (loved ones, healthcare providers, sponsor, employer, etc.)
- Two (2) suitcases filled with items outlined below
Keep in mind, space in your room may be limited, so please do not overpack.
You will have access to our washers and dryers, so 2 week’s worth of clothing should be enough.
Be sure to pack clothes that are easy to wash and dry and avoid bringing items that need special cleaning such as dry cleaning. Note that upon admission, your clothes will be placed into a high heat dryer for 30 minutes for infection control.
The following clothing items are approved. Clothing referencing drugs, alcohol, sex, gambling, or gangs is strictly prohibited.
